The MKA membership application consists of the following documents:

1. Membership Application Form - REQUIRED for all members. Only one required per family.
2. Agreement of Release - REQUIRED for all members. Only one required per family.
3. Medical Form - REQUIRED for each driver. Either a FULL medical form (provided by Physician) or a SELF-DECLARATION (self signed statement).
4. Parental Consent Form (ENGLISH) - REQUIRED for all drivers under 18 years of age.
5. Parental Consent Form (FRENCH) - REQUIRED for all drivers under 18 years of age.
6. New Kart Racer Test - REQUIRED for all first year drivers.
7. Proof of Birth - REQUIRED for all drivers under 18 years of age (photocopy of legal document displaying birth date, i.e.: birth certificate, passport).
8. Proxy Form - OPTIONAL form. Only one required per family.
9. Driver Profile - OPTIONAL profile. One profile per driver.

All completed applications are to be provided to the MKA Secretary.
